Add the name of your child’s center and take a moment to share how Head Start has made an impact on you and your child’s life.

Steps to Complete the Task:
Step 1: Take out your phone.
Step 2: Scan the provided QR code.
Step 3: When the window opens, it will prompt you to wish Head Start a happy birthday.
Fill in the required information.
Type the message: "Happy Birthday Head Start from PCI!" You can include details on how Head Start impact so many lives.
If you attended Head Start, check yes or no.
Then, hit Submit.
